The AMPLIFI team continues to bring live additional health systems within our project scope. Of the technically eligible sites, 35% have been integrated, 28% are in active implementations and 33% are undergoing preparations and planning for implementation.

Transitions of Care: Traverse Exchange Canada

Effective transitions of care are crucial for healthcare providers to improve patient outcomes and satisfaction. Utilizing electronic platforms, patient portals, and reliable vendor partnerships can create visibility, strengthen relationships, and improve engagement along the care continuum, ultimately leading to a positive patient experience.

The MEDITECH collaborative in partnership with Health Gorilla, are actively working to create this level of interoperability and transparency between participating health organizations in Ontario.  The Traverse Exchange Canada, will enable clinicians to access relevant patient health information from various sources, offering a comprehensive insight into patient needs.

Tim Pemberton, CIO and Chief Privacy Officer at Queensway Carlton Hospital in Ottawa, envisions that this work will limit the need for front line staff to gather information from numerous sources and piece together a patient history.

Initially available to MEDITECH customers in Ontario, Traverse Exchange Canada will later expand to include other vendors' systems and connect to jurisdictional systems nationwide, promoting data sharing and personalized care while ensuring patient privacy and security.
