Project Update:
Team AMPLIFI congratulates the TransForm Shared Service Organization (TSSO) hospitals on their bi- directional AMPLIFI Go- Live. Windsor Regional Hospital, Chatham-Kent Health Alliance, Hotel Dieu Grace Healthcare, and Erie Shores HealthCare are now able to bi-directionally exchange patient health information between their Oracle Health HIS and their integrated regional network of Long-Term Care homes, allowing hospital clinicians to view Long-Term Care patient information. The TSSO hospitals have already been contributing patient health information to the Ontario eHub HIE since March 30th.
Monthly expansion efforts are underway to bring live additional long-term care homes associated with our live health systems. The team has been actively engaging facilities that are in post go-live operations and continues to gather feedback on change management and training resources. In the past year, Project AMPLIFI has supported the transfers of 8994 residents, allowing clinical documentation to be seamlessly exchanged between the health system and the long-term care home.
Updates from PointClickCare
PointClickCare is pleased to announce that reason for transfer will now be included in the Continuity of Care Documents (CCD) outbound to hospitals from the long-term care homes. This means that when a patient is discharged from the long-term care home to the hospital, the reason for transfer will be immediately visible for the emergency room staff so they can quickly assess and successfully treat the long-term care resident.
On- Site Visits
We are thrilled to announce that as a part of the comprehensive training and change management plan for Project AMPLIFI, the project team is pleased to offer on-site support at LTCHs live- on integration, the session will be 1 hour in length.
The purpose of these on-site meetings is to:
- Review key workflows
- Identify any barriers or challenges to adoption
- Discuss opportunities and strategies to facilitate a successful implementation

Benefits Realization & Evaluation
As part of the funding commitment from the Ministry of Health and Ministry of Long-Term Care, Project AMPLIFI is completing a fulsome Benefits Realization and implementation evaluation of the clinical data exchange solution, including the assessment of provider perspectives.
Each Long-Term Care Home will be engaged to facilitate data collection to assess the impact of the solution on staff and health care provider satisfaction and perspectives.
Research Ethics approval for the data collection has been obtained for all Long-Term Care Homes under the Veritas independent research ethics board.
Data collection agreements and the collection process will be managed by Project AMPLIFI, who will connect with each organization or Long-Term Care Home to initiate the process.
