In the past year, Project AMPLIFI has supported the transfers of 13827 residents, allowing clinical documentation to be seamlessly exchanged between health systems and their network of long-term care homes.

Project AMPLIFI at the Ontario Long-Term Care Association – This is LTC 2023 Conference

The Project AMPLIFI team had the pleasure of participating in the OLTCA – TiLTC conference in Toronto this past month. It was great to connect and learn with stakeholders from across the province, all working to improve the quality of life and quality of care for our LTC residents.

The Project AMPLIFI team was delighted to speak with individuals representing long- term care homes across the province. It was a great way to garner feedback and facilitate further discussion about the continued change management and training support that the team offers such as on- site visits to long- term care homes and post go-live virtual sessions held monthly.

As part of the funding commitment from the Ministry of Health and Ministry of Long-Term Care, Project AMPLIFI is completing a fulsome Benefits Realization and implementation evaluation of the clinical data exchange solution, including the assessment of provider perspectives. Conference goers who work at long- term care homes that have been live on Project AMPLIFI for at least 3 months were invited to complete a short survey focused on assessing the impact of the solution on staff, including health care provider satisfaction and perspectives.

Overall, the conference was a great educational opportunity to share more information about Project AMPLIFI with leaders in the long- term care space. Networking with industry professionals and collaborators allowed the team to gain valuable insight from stakeholders who are focused and passionate about creating more seamless transitions of care for long- term care residents across the province.

Hospital and Long-Term Care Home Engagement Sessions

The Project AMPLIFI team is hosting Hospital and Long-Term Care project engagement & networking sessions as part of the MEDITECH hospital/cluster integration plans. The goal of these sessions is to both connect Hospital and LTC clinical leadership to promote networking, and to provide them with an overview of Project AMPLIFI to help ensure they have awareness of the project and help prepare them to engage with front-line staff.

 We host three one-hour sessions per cluster and include information on the project background, implementation scope and timelines, a high-level hospital and LTC workflow overview, change impact assessment discussion, and available training and change management resources.

During the months of October and November we are facilitating these sessions for the CHAMP Cluster Hospitals, Mental Health Cluster Hospitals, Holland Bloorview, Halton Healthcare and their local networks of LTCHs. More regions to follow as we approach their anticipated go-live dates.
